SCOPE AND AIM: On 1st April 2013 the responsibility for public health services within the County of Dorset transferred from the NHS to the local authorities of Dorset County Council, Bournemouth Borough Council and the Borough of Poole, all working together as Public Health Dorset with Dorset County Council being the host authority. Dorset County Council, on behalf of Public Health Dorset, is establishing a dynamic purchasing system [DPS] to commission residential substance misuse services. The DPS will consist of following Lots and Tenderers may apply for admission to one or both lots. Lot 1 – Dedicated Residential Detoxification with 24/7 nursing cover and medical support located in regions of South West England or South East England or London - A residential setting for detoxification and psycho-social support for the duration of the individual’s stay. The treatment should be appropriate and meet the need of the individual. This setting could be within a dedicated specialist setting or acute setting however it must align to current market rates for such services Lot 2 – Dedicated Rehabilitation with or without detoxification located in England or Wales - A residential setting that provides accommodation and a structured care planned programme of psycho-social support. This may include Detoxification. This setting is suitable for service users who have been identified as requiring increased support which may be out of area nearer family support Organisations that submit an Indicative Tender which meets the minimum selection criteria shall be admitted on to the Dynamic Purchasing System and shall be invited to submit Tenders [Invitation to Tender] for specific services when these requirements are identified DPS PERIOD: It is the intention for the DPS to be open for up to 4 years. NHS Dorset Clinical Commissioning Group is inviting expressions of interest from suitably qualified providers for the provision of non-clinical health coaching and social prescription in Primary Care for people with Long Term Conditions and Carers. The non-clinical health coaching and social prescription service for people with long term conditions and carers consists of the provision of three distinct but integrated service areas; 1. social prescription link worker/voluntary sector coordination 2. non-clinical health coaching 3. online digital support. Building on the outcomes of the NHS Dorset CCG clinical services review and reflected in the Dorset Sustainability and Transformation Plan, this service seeks to empower patients with long term conditions to manage their own health and wellbeing throughout their pathway of care as an integral part of the wider Personalised Care agenda for Dorset. It will enable a new way of working to be embedded within Dorset, drawing on core skills developed for the health and care workforce alongside that of Primary Care Home developments and The GP Forward View (2016) which identifies support for personalised care and support planning via self-management, social prescribing and active signposting as three of the 10 High Impact Actions, to release time for care. The service will be provided around a Primary Care/ Locality multidisciplinary team model of care to avoid duplication of services and complement existing care and support offers, with the added value of linking people to their own communities using voluntary sector organisations. It will enable GPs to access practical, community-based support for their patients, including access to advice on employment, housing and debt. NHS England Next Steps on the Five Year Forward View (March 2017) restated the importance of personalised care in meeting the triple aim of improved health and wellbeing, better care and greater value for the public pound. The service will support patients with long term conditions and carers with their non-medical needs, working in partnership with the wider care system across statutory and non-statutory services in primary and secondary care alongside community and voluntary services within each Dorset locality.
