The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO) 18, intends to issue a Request for Proposal for an Indefinite Delivery, Indefinite Quantity (IDIQ) contract for Ground Ambulance Services. This is a pre-solicitation notice for commercial items prepared in accordance with the format specified in the Federal Acquisition Regulation Subpart 5.203(a), as supplemented with additional information included with this notice. The acquisition will be a set-aside 100% for small business in accordance with FAR 19. The North American Industry Classification Code (NAICS) is 621910, Ambulance Services, with a small business size standard of $15 million. The period of performance will be for one (1) Year from date of award. The Contract shall furnish 24 hours per day, seven (7) days per week, ground ambulance transport services for the New Mexico VA Health Care System. The minimum qualification requirements are listed below to assist you in determining your qualifications for this solicitation: Response Time 1. Emergent Ambulance Services Contractor shall commence transportation of the patient within thirty minutes of notification of patient pick-up from the authorized Government Employee. 2. Non - Emergent Ambulance Services If transportation services are pre-arranged or scheduled in advanced, the Contractor shall commence transportation of the patient within ten minutes of the pre-arranged or scheduled time for patient pick-up. Otherwise, the Contractor shall commence transportation of the patient within thirty minutes of notification from the authorized Government Employee. The contractor shall be notified at least 24 hours in advance of the desired pick-up date and time. Operational Time Requirements: The Contract services shall be operable and available for use 100% of the time in a 24 hour 7 day week time period. Downtime will be computed from notification of problem during normal work hours. Operational Uptime will be computed during a month long time period. Repeated failure to meet this requirement can subject the contractor to DEFAULT action. Hours of Coverage: Ambulance services shall be available upon request twenty-four (24) hours a day, three hundred sixty-five (365) days per year, including all National Holidays. Note: Also included would be any other day specifically declared by the President of the United States to be a National Holiday. Contractor Personnel: Contractor personnel assigned to perform services under this contract shall meet or exceed all current City, County, State and Federal licensures, certifications and training relative to the function they are to perform under this contract. Minimally, included, would be re-certification(s), continuing education and/or refresher courses. These requirement shall be maintained current throughout the life of this contract. Personnel certifications and or documentations shall be presented to the CO upon request. Contractor Personnel Security Requirements All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA. Pre-Solicitation Notice Solicitation #VA258-15-Q-0495, Automated Tube Labeler, Raymond G. Murphy VA Medical Center. The New Mexico VA Healthcare System (NMVAHCS), Albuquerque New Mexico, intends to issue a Request for Quote (RFQ) solicitation requesting quotes to provide all equipment, supplies and services included in the statement of work. Summary of requirement: The Department of Veteran Affairs (VA) Cooperative Studies Program Clinical Research Pharmacy Coordinating Center (CSPCRPCC) has a requirement to purchase an automated tube labeler/packer for the labeling of bulk plastic tubes. The automated tube labeler shall have the capability to take bulk plastic tubes and label tubes in a predetermined location with pre-printed labels. The automated tube labeler shall orientate tubes in the same orientation prior to being labeled. The unit must also be able to label tubes in a pre-determined location with +/- 1 mm of accuracy. The accuracy of the label placement is critical to not cover the lot number or expiry dating on the tube. The machine shall have the capabilities of reading the tube for label skew and barcode readability. The tubes shall then be transported to a grouping station where tubes will be stacked into a tray (provided by the selected vendor) and transported down a conveyor for stacking. The contractor shall provide the automated tube labeler, installation, training, validation documents and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. The International Organization for Standardization maintains the ISO 9001 standard, and conducts periodic updates to improve or clarify quality standards. The ISO 9001 was updated in September 2015 and the new standard is currently ISO 9001:2015. The organization also approves certification bodies to administered accreditation to the ISO standard. CSP (Albuquerque, Boston, Hines, Palo Alto, Perry Point, and West Haven) contracted with an accreditation body and is currently ISO 9001:2008 registered. In addition, Albuquerque is also registered to a Good Manufacturing based ISO standard, 15378:2011. ISO 15378:2015 was updated and reissued in October 2015. Registration to an ISO 9001 standard certifies that formalized quality management processes are being employed consistently to meet customer and regulatory requirements; those processes are continually improved to enhance customer satisfaction; and a third party has validated those processes. CSP's quality program is surveilled and monitored to the ISO standard annually. CSP must now update their Quality Management Systems (QMS) to adhere to the new 2015 standards by 2018. Moreover, the Registrar's auditor must see that CSP is making a good faith effort and planning to conform to the new standard in the interim years. As part of that effort, CSP needs access to both the new and old standard in an electronic format to ensure proper Center wide analysis and updates to current system processes. While individual copies could be purchased for a few, vendors prohibit sharing. All employees should be able to access the standard for robust deployment of the QMS. A multi-user site license for ISO 9001:2008 and ISO 9001:2015 posted on a CSP SharePoint site is more cost effective. 4. SCOPE.
