3.1. The contract is for the provision of cleaning and disinfection of premises and cleaning outdoor areas Children's University Hospital in Krakow, ul. Wielicka 265, 30-663 Kraków hereinafter referred to as PURCHASER 3.2. The required minimum payment period is 60 days. 3.5. Contractor uses toilet paper dispensers, paper towels, liquid soap that are owned by the Purchaser. The Contractor will be required to complete the missing and damaged dispensers contractor is responsible for maintaining the inventory of functional dispensers and hygiene measures. After completion of the contracts listed by the Contractor dispensers become the property of the Employer. 3.6. The Contractor shall at his own expense detergents, disinfectants needed to perform the services at issue. 3.7. The Contractor shall at its own cost bags used as bin liners for municipal waste (black) and plastic bags used as contributions to municipal waste collected separately (blue) and the corresponding contributions bags as containers for medical waste including hazardous medical ( red). 3.8. The Contractor shall at his own expense hygiene such as soap, liquid soap type Faren for dental treatment, toilet paper, paper towels. 3.9. Contractor to perform the service which is the subject of proceedings shall bear the cost necessary equipment and facilities for cleaning and disinfecting. 3.10. The Contractor shall maintain procedures for cleaning and disinfection in accordance with the rules laid down for the premises of infection in high-risk surgeries: dental, outpatient surgery, endoscopy lab, oncology-surgical point of collection materials for research, emergency room, other offices if they are performed invasive procedures and the immediate removal of any organic impurities, 3.11. The Purchaser requires that the service provided by the Contractor complies with quality management system ISO 9001 or equivalent. 3.12. Disinfectants preparations for routine surface disinfection must have spectrum B, V 3.13. Disinfecting preparations for disinfection in high-risk areas and are used to remove organic contaminants must have full spectrum (B, V, F, TBC). 3.14. Disinfectants preparations must be stored in original packaging in a cool, dry, inaccessible to unauthorized persons, under confinement label in Polish. 3.16. The duration of action of disinfectants to surfaces does not exceed 15 minutes. 3.17. Detergents, disinfectants and preservatives, for performing the services must be according to their kinds, the following documents: - MSDS - entry in the Register of Medicinal Products, Medical Devices, Biocides / notification to the register of medical / permission of the Minister of Health on turnover biocidal product - certified for cleaning agents and preservatives 3.18. All the measures and facilities must meet the requirements of the Act of 30 August 2002, on Conformity Assessment (Journal of Laws of 2002 No. 166, item. 1360) and the Specific Regulations issued under the above Act. 3.19. The Contractor shall provide new wiper-slip absorbing moisture and dirt in the average amount of 6 units, including 4 units of 1m x 2,5 m; 2 pieces of 1m x 4m 3.20. Contractor is obliged to refund of the purchase inner containers for selective collection of municipal waste contractor who previously performed a cleaning service. 3.21. Before signing the contract, the Contractor agrees and shall provide a list of measures which will carry out the service to approval by the Hospital Infection Control Team. 3.22. List of surfaces and spaces covered by the contract is contained in Annex 4 to the ToR 3.23. Scope of action
