Providers are invited for the supply and delivery of provision of support for joint working between GP practices in Horsham & Mid Sussex CCG. We recognise that our current model of service delivery is not sustainable – demand for health and care services is growing as our population lives longer and we know that the cost of meeting this demand is rising more quickly than we can afford. Patients, and in particular those with long term conditions, often find their experience of care disjointed and uncoordinated. Looking to the future in Horsham & Mid Sussex we know we will have a larger population and one that is living longer with complex needs. At the same time locally our general practices and community services are increasingly struggling to meet the current demands. Our aim is to deliver a new model of care that addresses the three ‘gaps’ identified in the Five Year Forward View and delivers high quality, accessible care to our population, moving from a service that is primarily reactive and bed based, to one that focuses on keeping people well and treating them proactively in the community when they do become ill and which that results in better health and wellbeing for local people and better value for money from health and social care services. Overview of Services below: Lot 1 Provision of a Community Equipment and Installation Service for adults living in East Sussex or registered with an East Sussex GP East Sussex County Council Adult Social Care and East Sussex Clinical Commissioning Groups (the Commissioning Partnership) are retendering their Community Equipment Service and Installation Service as one lot (Lot 1). Both these services support the whole systems policy to help more people live independently in the community, reduce acute bed care, facilitating safe and timely discharge and support admission avoidance strategies. Lot 2 Provision of a Managed Telecare and Assistive Technology Service for adults living in East Sussex. East Sussex County Council Adult Social Care is re-commissioning a new service model to provide our countywide Telecare service. The purpose of the Managed Telecare and Assistive Technology Service is to pro-actively support adults to maintain and develop their independence through the increased use of Telecare and AT as part of the Departments move towards reablement. Service components include: — a cost-effective managed installation service, supported by an online ordering facility, — underpinned by an efficient call monitoring service that enables the safe care and early detection of increased client need, pro-actively supporting clients to increase their ability to self-care, — and supports Adult Social Care prescribers to increase client independence and reduce the need for other forms of support.

Brighton and Hove Clinical Commissioning Group (CCG) and Brighton and Hove City Council (BHCC) wish to procure a Community Short Term Beds Service for residents in the Brighton and Hove area. Brighton and Hove Clinical Commissioning Group (CCG) will be the Lead Commissioner. Brighton and Hove City Council (BHCC) will be an Associate Commissioner. The aims of the new service are as follows: • Reduce unscheduled admissions to hospital • Reduce delayed transfers of care from hospital • Reduce admissions to long term placements • Reduce demand for long term packages of care To achieve these aims the objectives of the service are to: • Increase access to step up beds • Increase flexibility in eligibility criteria to support the system where required • Reduce assessment and admission timescales • Reduce average lengths of stay • Increase focus on short term goals to enable people to return home • Increase engagement with service users and carers in setting and achieving short term goals The Contracting Authorities are looking for providers with the necessary capacity and capability (or a demonstrable ability to provide the necessary capacity and capability) to deliver high quality, patient-centred and value for money services, delivered in a safe and effective manner and through a learning environment which may include the training of healthcare professionals. Contract Information The Commissioners have agreed to offer a 5 year contract initial term with a single option to extend for a further 2 years (5 + 2 = 7 years maximum duration). A financial envelope of £5.5m per year is available for the 7 year maximum duration of the contract. The maximum total financial envelope is therefore set at £38.5m. To support the procurement, Brighton and Hove City Council have two buildings that are available for lease or purchase. The properties will be available at a fixed rental or sale price for all interested providers. Alternatively, Providers are free to source their own property - or use a combination. Contract award is expected in December 2016 and the service start date is 1st April 2017.
