Educational Consultant Service Contract Scope of Work Nenahnezad Community School is in need of Educational Consultant Services to provide Professional Development, on-site coaching, technical assistance, and leadership development. The consultant will provide 20 days of onsite visits from the date of award to June 30, 2017. Trainings will guide the school through implementation of the State Standards for Mathematics and Literacy; provide professional development to focus on the goals of the Navajo District Plan, such as ELL strategies, student engagement, lesson planning, curriculum mapping, and effective assessment strategies. To accomplish these goals they will make classroom observations with feedback, provide coaching for teachers, instructional coaches and the principal, facilitate lesson planning, planning for the three Tiers of instruction and data analysis sessions. In addition the consultant will monitor teacher implementation of Common Core instruction, provide customized support to increase outcomes for students, and provide data that teachers can use with their professional portfolios and the EPAP process. Finally the consultant will debrief with teachers and the principal to develop next steps, and provide detailed reports to principal and coach. The evaluation will be completed at the school by the government representative. This form is the only performance evaluation that will be accepted by the Contracting Officer. The government representative will review the performance evaluation with the Contractor, record Contractor comments, and obtain Contractor signature acknowledge in completion of the evaluation. SECTION M FAR 52.212-02: Evaluation -Commercial Items: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The quote should include, but not limited to the following: a) Knowledge and understanding of the requirements outlined in the SOW. b) Understanding of general and local condition which can affect the SOW. c) Logical sequence of steps and or specific techniques to accomplish all required work specified in the SOW. d) Capability to produce deliverables in response to the SOW. FACTOR II - Management Capabilities for a Consultant Services, will work with student, in accordance with the Statement of Work. Each offeror shall submit a plan consisting of a narrative explanation of their quote on-site approach to project management and coordination; and cost control measures on change orders and how they are quoted and tracked. Each offeror will be required to provide the relevant experience and knowledge of key individual(s) of the project team who will be assigned directly to the project. This factor will assess the quoted key personnel qualifications. The assessment will consider whether the quote provides an adequate number of appropriately qualified personnel to meet the technical, management and support requirements of the SOW. The Offer should include, but not be limited to the following description: a) Describe the method and criteria you plan to use to select qualified subcontractors to ensure an appropriate level of experience and expertise for the project. b) Describe your project schedule and plan indicating where you will start the project and how you plan to proceed to completion of the work as identified in the statement of work. Your projected schedule of the project; shall include steps within each phase and their estimated duration time. Project Schedule should note critical path. c) Submit a resume along with a letter of commitment for all key personnel. FACTOR III - Past Performance for a Consultant Services, will work with student, in accordance with the Statement of Work. Please provide a list of contracts to include the contract number, POP, point of contact information and the description of the requirement. "Successful past performance on similar projects, to include but not limited to timeliness of performance and customer satisfaction". FEDERAL TRAVEL REGULATIONS: Travel cost incurred for transportation, lodging, meals, and incidental expenses (as defined in the regulation cited in FAR 31.205-46) shall be considered to be reasonable and allowable only to the extent that they do not exceed on a daily basis the maximum per diem rates in effect at the time of travel as set forth in the Federal Travel Regulation. The consultant will be reimbursed for travel to provide support at a Government site or other site as may be specified and approved by the COTR under this effort. All travel shall be approved, by the COTR, prior to commencement of travel. Consultant shall provide supporting documentation and a detailed breakdown of incurred travel costs with each invoice. BACKGROUND INVESTIGATION: In accordance with 25 U.S.C. 3201 (P.L. 101-630), Indian Child Protection and Family Violence Prevention Act; 42 U.S.C. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duties and responsibilities involving regular contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Accordingly, all such positions require a favorable screening (applicant and suitability) determination made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program; and a favorable background investigation determination conducted by the Office of Personnel Management (OPM). A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of the Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract. Pending completion of a background investigation by OPM, the Contractor who has been granted a favorable screening (applicant and suitability) determination must be within the sight and under the supervision of a Government employee who has had a completed background investigation (42 U.S.C 13041 (3)) during contract performance until a final background investigation determination has been made. Prior employment with the Federal Government, including BIA, OIEP or the Bureau of Indian Affairs, does not assure that the appropriate screening or investigation was conducted nor does it alleviate or guarantee a favorable screening (applicant and suitability) determination or background investigation. Prior to an award or during the 60 day period discussed in paragraph two, it is solely the company, individual or contractor's responsibility to contact the BIA, OIEP Personnel Security and Suitability Program at 505-563-5287/5288/5289/5294 in order to obtain a complete security package, which includes but is not limited to appropriate investigative forms and/or inquire about their previous investigation; and, obtain a screening (applicant and suitability) determination, and subsequent final background investigation. If an unfavorable screening (applicant and suitability) determination is made, a purchase order shall not be awarded to the individual who received an unfavorable screening or to the company who has proposed to employ the individual to perform the work. The individual and or the company who proposed to employ this individual may be determined to be non-responsible if an unfavorable screening (applicant and suitability) determinations is made, which will make them ineligible for purchase order award. If an unfavorable final background investigation determination is made, the purchase order may be terminated and the employee in question shall not be authorized to provide service to the BIA, OIEP, if the services involve contact with or regular control over Indian children. Award will be made to the lowest priced offeror, whose quotation is conforming to the requirements herein, will be most advantageous to the Government, and is fair and reasonable. Requirements. The Medical Support Systems, Project Management Office (PMO-MSS) of the U.S. Army Medical Materiel Development Activity (USAMMDA) has a requirement to obtain a commercially available CPPW. This Statement of Need shall facilitate purchase of the CPPW to replenish stock. The proposed contract shall be Firm Fixed Price consisting of one base CLIN and two separate option quantity CLINs for the base year. 2. Background. The CPPW is used to protect patients who are incapable of donning individual chemical protective over garments. The CPPW will enable patients to be transported and treated safely during evacuation through chemically contaminated areas. It is used in forward areas on the battlefield such as Battalion Aid Stations and Medical Companies. The CPPW is also used with patients at Combat Support Hospitals. 3. Period and Place of Delivery Requirements 3.1. The proposed Delivery Date for the initial purchase of the Chemical Wraps is estimated at no later than 60 days after award. Earlier delivery is desired. The Period of Performance for the Option Quantity CLINS is estimated at 06 July 2016 - 05 December 2016. 3.2. Place of delivery. The primary delivery location will be at the U.S. Army Medical Materiel Agency Stock Maintenance Division, c/o U.S. Army Medical Materiel Development Activity (USAMMDA), BLDG. 1160, 6149 Wardleigh Road Bay 2, Hill Air Force Base, UT 84056-5948. 4. Minimum Essential Characteristics. 4.1. Protective Characteristics. 4.1.1 The CPPW shall be capable of providing respiration and percutaneous protection for unmasked, uncontaminated patients for a minimum of six hours after initial exposure to known potential chemical warfare agents that could be employed in vapor, aerosol, liquid, or thickened liquid form. The CPPW fabric, at minimum, must demonstrate resistance to permeation (liquid challenge) for two chemical agents: Soman (GD) and Sulfur Mustard (HD). Vendors must provide test reports or evidence of swatch testing of materials used in the fabrication of their candidate product. Aerosol Vapor Liquid Assessment Group (AVLAG) TOP 8-2-501 references standards for swatch testing of material. The test report must be provided from an independent lab or testing activity. 4.1.2 The upper surface of the CPPW will be water repellent so that the protective characteristics will not be degraded by exposure to moisture such as mist, rain, snow or saltwater spray. The bottom surface/material of the CPPW will be totally impervious to moisture, including liquid chemical agents. 4.1.3 The protective characteristics of the CPPW shall not be degraded when body excreta such as blood, urine and feces are brought into contact with the interior surfaces of the wrap. 4.2. Physical Characteristics. 4.2.1 The CPPW shall be a disposable, expendable item of medical supply and weigh no more than nine pounds in its packaged configuration, excluding the blower system, such as the Powered Air Purifying Respirator (PAPR). The current PAPR system used for this purpose is the C420 Blower and C2A1 Filter Canisters. Rechargeable batteries are required for use with the PAPR. 4.2.2 The CPPW shall be designed and manufactured in one size. It shall be large enough to completely encapsulate military personnel ranging from the 5th percentile female to the 95th percentile male, in accordance with MIL-STD-1472, and bear their weight without ripping or opening during movement. It shall have adequate space at the head to permit the use of a medical non-breathing mask, endotracheal intubation tubes, supraglottic devices, and cricothyroidotomy tubes. 4.2.3 The CPPW shall accommodate patients wearing current Army issue tourniquets, splints, backboard, and junctional hemorrhage control devices. 4.2.4 The CPPW shall be manufactured of materials which will not cause safety or health hazards to patients or using personnel. Vendors must provide safety or health hazard assessments (such as Material Safety Data Sheets (MSDS)) to indicate their product's assessment in this area. 4.2.5 Design will be such that air and carbon dioxide will be exchanged through the CPPW. Carbon Dioxide buildup shall be less than 3% (by volume) over a 6 hour test period in other than hot climatic regions as defined in Army Regulation (AR) 70-38. The oxygen level shall not fall below 18% (by volume) over the 6 hour test period in other than hot climatic regions as defined in AR 70-38. The hot climatic region is characterized by two daily weather cycles, one representing the highest temperatures likely to be found anywhere in the world and the other representing extremely high dew points. In hot climatic regions the time limit used will be determined during development, using healthy subjects. 4.2.6 A transparent window shall be located at the head-end of the CPPW to allow full head visibility of any size patient (5th percentile female to the 95th percentile male). 4.2.7 The fastening devices of the CPPW shall permit medical personnel easy and quick access to patients. 4.2.8 The CPPW must have an easily accessible, transparent pocket on its upper side. The pocket shall be able to hold a Patient Evacuation Tag (such as a Department of Defense (DD) Form 1380). 4.2.9 The CPPW shall be designed or marked in such a manner to prevent inadvertent reversing of top/bottom and ensure proper use. 4.2.10 The CPPW shall permit a patient to extricate him/herself without extreme effort within a time limit of 1 minute. It is recognized that in some situations the patient's injuries or sedation might make this impossible. 4.2.11 The CPPW shall allow for passage of intravenous/intraosseous infusion devices and respiratory tubing. Passageways should not exceed .59 inches in diameter. Two such passageways are required. 4.2.12 The CPPW shall have some type of carrying handles. The handles must be capable of holding military personnel ranging from the 5th percentile female to the 95th percentile male in accordance with MIL-STD-1472. 4.3. Packaging and Storage Characteristics. 4.3.1 Internal packaging shall be hermetically seam-sealed using available seam-sealed technologies for chemical protective items. 4.3.2 External packaging must be impervious to rain, dust and saltwater and shall permit rapid access to and employment of the wrap by medical personnel. Packaging materials must be impervious to all known threat agents for a period of time which is sufficient to permit decontamination by weathering. 4.3.3 The CPPW will be designed for storage in climatic design types hot and basic in accordance with Army Regulation 70-38 with no degradation of performance for a minimum period of 10 (ten) years. 4.3.4 Packaging dimensions shall not exceed 19.5 in long x 18 in wide x 6.5 in thick. 4.3.5 The blower system shall be able to pass limited rotary-wing Airworthiness Certification. 4.3.6 The government shall procure the CPPW as a kit comprised of 12 individual wraps. The kit shall include all the items (wraps, filters, blowers, batteries, etc.) necessary for each individual patient to have a complete CPPW system. The forecasted total procurement quantity is 492 CPPW Kits, which equates to 5,904 individual wraps. 5. Additional Requirements. 5.1 All items required to use in the kit in a deployed military environment shall be identified by part number, weight and cube to enable the establishment of National Stock Numbers (NSN). 5.2 Vendors shall include the following items with proposal: 5.2.1 Equipment Support Kit Lists. 5.2.2 Equipment Repairable Items List. 5.2.3 Equipment Commercial Service Manual. 5.2.4 Equipment Commercial Operation/Instruction Manual. 5.2.5 Equipment Consumable/Durable Support Items List. 5.2.6 Detailed Material Safety Data Sheet (MSDS) for all materials used in manufacturer and/or production of the CPPW.5.2.7 Certified report of swatch testing of materiel. 5.3 Any modifications to the Chemical Patient Protective Wraps during the Performacne of this contract that will affect future wrap deliveries, Contractor shall notify the Contracting Officer Representative (COR), Contracting Officer (KO), and Contract Specialist (CS) within 30 calander days of the modification completion. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. (b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show-- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. (d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during pre-award testing. (e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately. (f) Late submissions, modifications, revisions, and withdrawals of offers: (1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due. (2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and-- (A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or (B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or (C) If this solicitation is a request for proposals, it was the only proposal received. (ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted. (3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel. (4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume. (5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. (g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://www.acquisition.gov. (l) Debriefing. If a post-award debriefing is given to requesting offerors, the Government shall disclose the following information, if applicable: (1) The agency's evaluation of the significant weak or deficient factors in the debriefed offeror's offer. (2) The overall evaluated cost or price and technical rating of the successful and the debriefed offeror and past performance information on the debriefed offeror. (3) The overall ranking of all offerors, when any ranking was developed by the agency during source selection. (4) A summary of the rationale for award; (5) For acquisitions of commercial items, the make and model of the item to be delivered by the successful offeror. (6) Reasonable responses to relevant questions posed by the debriefed offeror as to whether source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities were followed by the agency. ADDENDUM TO 52.212-1 ADDITIONAL INSTRUCTIONS TO OFFERORS 1. TECHNICAL QUESTIONS AND PRE-PROPOSAL DISCUSSION Questions concerning the solicitation are due no later than 22 July 2016 at 1:00 P.M. INSTRUCTIONS FOR THE PREPARATION AND SUBMISSION OF PROPOSALSFailure to submit all documents concurrently and in accordance with these instructions may render a proposal unacceptable. PROPOSAL EVALUATIONTo aid in evaluation, proposals shall be practical, specific complete, and logically assembled. The proposal shall be in a clear, concise format satisfying all requirements of this solicitation. It shall be in sufficient detail to demonstrate a complete understanding of the work to be accomplished. 4. TECHNICAL INSTRUCTIONS:Contractors shall provide sufficient written technical documentation to allow for a thorough evaluation of their quote. Additionally, all sections of the Offeror's Technical Quote shall make direct reference to the Statement of Need (SON) and Minimum Essential Characteristics (MECs). The Quote shall be presented in a clear and logical order. Statements such as "will comply", "meets the intent of", or "we intend to meet the requirements" and the like, will not suffice for evaluation purposes. The Offeror shall demonstrate that it has a clear understanding of the requirements outlined in the SON and MECs and the ability to meet the requirements in order to be considered for award. Technical quotes shall include sufficiently detailed information to enable evaluation based on the two (2) factors listed below: 1. Technical Acceptability -The Offerors proposal shall contain sufficient document that demonstrates they have a clear understanding of the requirements outlined in the SON, MECs and the Solicitation. The Offeror must also provide sufficient documentation to clearly demonstrate that their "Or Equal" product meets the current desired Brand specifications. It is the Offerors responsibility to provide this information to the Government. In addition to demonstrating a full understanding of the requirements outlined in the SON and MECs, the Offeror's proposal should include all required documentation detailed in the list below. The Offeror may indicate if this information is included in the required documents listed as bullets "b" through "h" of this section (in which case, a separate list need not be provided).The Offeror may also indicate if any of the following are not applicable to the proposed system. Past Performance -The contractor shall describe its past performance for up to five (5) but no less than three (3) contracts it has held within the last three (3) years that are for the same or similar supplies to that which is detailed in this solicitation, or affirmatively state the contractor possesses no relevant past performance directly related to or similar to this requirement. Contractors who submit past performance references for similar contracts shall provide a detailed explanation demonstrating the relevance of the contracts to the requirements of the solicitation. The Government reserves the right to contact any reference. A contract of similar nature is a contract of comparable risk - functions, responsibilities, and control exercised by the Contractor which is essentially the same as required under this solicitation. 3. SMALL BUSINESS SUBCONTRACTING PLAN (SBSP)The SBSP shall be a separate document from the Small Business Participation Plan (which is part of the Phase Two Technical Proposal). All Offerors, except U.S. Small Businesses, shall submit a SBSP that addresses all factors in FAR 52.219-9 (Dev) FAR 52.219-9 Alt II (Dev) and DFARS 252.219-7003 (Dev). The SBSP will be used to determine contractor responsibility. In addition to the SBSP, Offerors shall certify compliance with FAR Clause 52.209-6 and provide sufficient information for the Government to determine responsibility of its proposed subcontractors. The Small Business Subcontracting Plan is not a requirement for evaluation in source selection, but rather, a requirement for award to a large business and will be incorporated into any resultant Contract. Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. The US EPA, Office of Air and Radiation (OAR), Office of Transportation and Air Quality (OTAQ), Assessment and Standards Division (ASD), has a need for contractor services for a variety of technical analyses and support activities such as services for various analyses, modeling, and other activities associated with the development of regulations for mobile source emissions. It is anticipated that any future contract for these services will be a Cost-Plus-Fixed-Fee (CPFF), Level-of-Effort (LOE) type contract with five (5) one-year periods, each with LOE of approximately 128,000 hours. The applicable NAICS code is 541620 (Environmental Consulting Services) with a size standard of $15 million. The successful contractor of any resultant contract will be expected to provide all necessary personnel, equipment, and services necessary to conduct the research activities delineated by the Performance Work Statement (PWS). The technical and scientific services to be provided under this contract will support the Agency in its development of regulations for motor vehicle, aircraft, marine vessel, locomotive, and non-road equipment manufacturers; engine manufacturers; and producers and distributors of fuel/fuel additives. It is considered to be in the best interest of the Government to exclude from award as a prime contractor or subcontractor those entities that are directly engaged in or that have a business, financial, or competitive relationship(s) with firms that are motor vehicle, aircraft, marine vessel, locomotive, or non-road equipment manufacturers; engine manufacturers; or that produce or distribute fuel or fuel additives. In addition, the Agency has determined that offerors that work directly for or that provide consulting or technical services to motor vehicle, aircraft, marine vessel, locomotive, and non-road equipment manufacturers; engine manufacturers; or that produce or distribute fuel or fuel additives may also have significant actual or potential OCOI concerns. Therefore, all offerors will be required to submit an OCOI plan identifying the projects they are working on and explain how potential or actual OCOI can be mitigated, avoided, or neutralized. In order to be considered for award, a determination must be made that the proposed plan adequately protects the Government’s interest. All offerors shall submit an OCOI plan that describes procedures the offeror uses to identify and report COIs. The solicitation shall include standard EPAAR provisions (EPAAR 1552.209-70 and 1552.209-72) which require that an offeror certify whether it is aware of any potential OCOI or disclose any potential OCOI, and provide disclosure statements describing such OCOI, which may impact their ability to impartially conduct work under the PWS. The standard EPAAR solicitation provision (EPAAR 1552.209-71) requiring a contractor to make full disclosure of any actual or potential conflict of interest that is discovered or arises after contract award shall also be part of the solicitation. Furthermore, the EPAAR clause (EPAAR 1552.209-74, Alternate V, Limitation of Future Contracting) shall also be included in the solicitation and prohibits the contractor, without the approval of the Contracting Officer, from entering into contracts with an individual, organizations, or their consultants, to provide services on or in support of the specific remediation equipment, technology, or process identified in a work assignment. I. INTENT A. The Robley Rex Veterans Affairs Medical Center has an open-ended AE Design requirement for professional Architect and Engineering services that requires the design of various Non-Recurring Maintenance (NRM), Clinical Specific Initiatives (CSI), Minor Construction projects and Medical Center funded projects. Design services to be provided are multi-discipline in nature and primarily focused on Architectural, Structural, Civil, Landscaping, Mechanical, Electrical, and Plumbing (MEP) Designs. B. Firm-Fixed-Price (FFP) Indefinite Delivery Indefinite Quantity (IDIQ) contracts resulting from this solicitation will include profit, overhead and hourly rates for anticipated disciplines and for use in negotiating FFP task orders. Profit, overhead, and hourly rates for anticipated disciplines may be further negotiated at the task order level, but may not exceed the negotiated hourly rates for the applicable contract performance period. The contracts will have a minimum guarantee of two thousand five hundred dollars and zero cents ($2,500.00) for the base period only, with a maximum ceiling amount of four million nine hundred ninety nine thousand nine hundred ninety nine dollars and zero cents ($4,999,999.00) over the life of the contract. The intent of this requirement is to make a minimum of no less than four (4) contract awards. D. The A/E firm must be capable of responding to and working on multiple task orders concurrently. II. SCOPE OF WORK UNDER THE CONTRACT A. The selected firm will be required to provide site investigations, schematics, design development, construction documents, detailed construction cost estimating, Independent Government Estimates (IGE), technical analysis, construction submittal reviews, design clarifications and record drawings for projects that require specialized design considerations, complex engineering solutions, specialized consultants, and the use of uncommon materials, and means/methods, in occupied federally owned or leased healthcare buildings. Other possible tasks required may include but are not limited to site investigation, code compliance reviews, technical design reviews, constructability reviews, envelope system integrity reviews, construction observation, construction inspection, historic preservation, historic structure reports, and space planning. B. The work to be performed under this contract is to provide specialized and innovative design-related AE services in support of the Veterans Affairs Mid-South Network's capital design and construction program mission. Disciplines and expertise required shall include healthcare design and construction, historic preservation, fire/life safety, architectural, civil engineering, structural engineering, mechanical engineering, electrical engineering, fire protection engineering, roofing design, construction inspection, scheduling, AutoCAD/Building Information Modeling (BIM) design, (the successful AE firm or team shall have experience with BIM in developing projects from its initial conception through completion of documentation) acoustical engineering, lighting engineering, interior design, surveying, geotechnical engineering, environmental engineering, certified industrial hygiene, recent experience in the design of contemporary, state-of-the-art medical facilities, with additional experience in vertical expansion/integration along with phasing of on-going operations, research facilities, long-term care-mental health and dementia, elevator and other disciplines and experts with specialized experience. C. All designs are required to achieve excellence in design. While these tasks, disciplines and expertise are needed for performance of task orders, other tasks disciplines and expertise that may be required are site development, architectural, fire protection, interior design, structural, plumbing, sanitary, HVAC, electrical, equipment, steam generation, steam distribution (outside), solid waste disposal system (including incineration), automatic transport, asbestos abatement, space planning, Critical Path Method (CPM), estimating, specifications and final bid documents. Therefore, adaptability is required and should be demonstrated. Expertise with environmental issues such as sustainability, energy modeling, alternative energy application and related items may be required with this contract. D. Typical construction projects that will result from the design efforts of the A-E may include repair and alteration/re-use of building interior/exterior spaces, systems and features. The majority of construction projects under this contract will have an estimated construction cost less than $5,000,000 dollars. This contract will require the A/E to perform, at minimum site development, architectural, fire protection, interior design, structural, plumbing, sanitary, HVAC, electrical, equipment, steam generation, steam distribution (outside), solid waste disposal system (including incineration), automatic transport, asbestos abatement, space planning, Critical Path Method (CPM), estimating, specifications and final bid documents. 1. DESIGN PROGRAMS a. DESCRIPTION OF WORK TO BE PERFORMED Preparation or review and analysis of design program(s) that delineate the functional and physical parameters, which define project specific requirements, and constraints that express the client's priorities. The design program is an ongoing process that must be revised if the project goals are changed. Development of the design program will be in accordance with Department of Veterans Affairs, Functional and Physical Design Program. b. DELIVERABLES The end product will be the design program submissions and/or evaluation of reports, documents, and cost estimates supporting the design program prepared by the AE. One reproducible original and two copies will be submitted to the Contracting Officer. 2. SCHEMATIC DESIGN a. DESCRIPTION OF WORK TO BE PERFORMED The preparation, planning, evaluation, review and recommendation of all architectural and engineering discipline calculations, specifications, reports, system analyses, drawings and cost estimates meeting the approved space program and Design Program in accordance with the AE Submission Instructions for Minor and NRM Construction Program, Program Guide PG-18 15, Volume C. Schematic drawings for construction projects are a means of identifying alternative approaches to correcting physical and functional deficiencies. The development of these schematic drawings is an interactive process between the VA Medical Center staff and the AE in order to produce a narrative and graphical description of possible program alternatives. b. DELIVERABLES The schematic design submission will meet the requirements of the distribution schedule in the PG-18 15, Volume C. 3. DESIGN DEVELOPMENT a. DESCRIPTION OF WORK TO BE PERFORMED The final approved Schematic Plans shall be the basis for the development of the Design Development phase. Any changes from these plans must be approved by the Veterans Affairs Project Manager prior to proceeding with the Design Development Drawings. The result of this phase will be a set of design documents defined to the point that no further functional decisions are required. All plans will be reviewed for functional and aesthetic relationships. b. DELIVERABLES The development of the design development submission will meet the requirements of the distribution schedule in the PG-18 15, Volume C. 4. CONSTRUCTION DOCUMENTS (CDs) a. DESCRIPTION OF WORK TO BE PERFORMED The Construction Document phase involves the production of complete drawings, specifications, and other documents necessary for the bidding and construction of the project, prepared from the approved Design Development documents. Also included at this phase are the final cost estimate, the final phasing plan, and the construction schedule. 1) It is the AE's responsibility to provide a quality set of documents. Related documents shall be complete, fully coordinated and ready for reproduction for contract. Prior to reproduction for issue for construction bids, make any changes to the documents identified as necessary during the final Construction Documents review conference with VETERANS AFFAIRS Medical Center staff. b. DELIVERABLES The construction documents submission will meet the requirements of the distribution schedule in the PG-18 15, Volume C. 5. CONSTRUCTION PERIOD SERVICES a. DESCRIPTION OF WORK TO BE PERFORMED The Construction Period Services, when required, are for review of contractor submittals, site investigations, technical advice, and closeout documentation. The Contractor shall provide construction period services which shall consist of submittal reviews and approval recommendations for products within the contract documents, site visit, quality control/quality assurance regarding project drawings and plans, attendance at project meetings with the construction contractor. Submittals may consist of cut sheets, manufacturers data/performance sheets, samples, and shop drawings. Submittal reviews may include compatibility analyses of different materials and recommendations associated with acceptance or rejection of alternate materials and products. Review duration will meet requirements established by the Contracting Officer. The contractor shall conduct site visits and participate in intermediate inspections as well as the final inspection of completed construction. The number of site visits and intermediate inspections to be conducted will be identified within the specific task orders issued. Construction period services shall also include the review and/or development drawings and cost estimates related to design errors/omissions and unforeseen conditions. b. DELIVERABLES - Services provided will meet the requirements established within the task order issued. Number of submittal copies returned and/or prepared will be in accordance with direction from the Contracting Officer. 6. CONSTRUCTIBILITY REVIEW a. DESCRIPTION OF THE WORK TO BE PERFORMED The Contractor will provide a detailed analysis of the construction documents to assure clarity, accuracy, coordination and completeness of the documents. The review will focus on the suitability of the drawings and specifications for ease of construction and the placement and maintenance of equipment. Review will identify conflicts and/or claims that may arise during construction. b. DELIVERABLES The Contractor shall provide report(s) evaluating the constructability of the contract documents to the Contracting Officer five (5) workdays prior to the Veterans Affairs Facilities Management construction document review. One reproducible original and two copies will be submitted as stated above. 7. SPECIALTY CONSULTANTS NOT AVAILABLE THROUGH THE DESIGN A/E a. DELIVERABLES Submission of reports and recommendations associated with design and construction activities and investigations will be accomplished on a schedule developed by the Contracting Officer. Submit one original reproducible and two copies of the report(s) to the Contracting Officer. All construction and investigation services may require both on site inspection and report preparation. When an agreement has been reached on services to be performed, labor rates for the multi-disciplines indicated on ***Exhibit A-Architectural/Engineering Fee Schedule, and overhead and profit percentage rates indicated on ***Exhibit B-Architectural /Engineering Overhead & Profit Rate Schedule shall be used for as applicable for the project design indicated by each Task Order. The hourly labor rates, the overhead and percentage fees shall be considered fixed pricing for the duration of this IDIQ and each option year exercised. D. The AE shall clearly state each discipline used in the AE's proposal in response to each Task Order issued by the Government. The Government and contractor shall negotiate G&A expenses, the total labor hours, materials and other direct expenses necessary to perform, along with the performance period (expressed in calendar days), measured from the issuance of the notice to proceed to the date for completion. The final price will include the fixed hourly labor rates for the applicable disciplines, the fixed overhead and profit percentage rates, as well as pre-negotiated direct and indirect expenses, G&A expenses. E. No work shall be performed on an individual task order until the Contracting Officer has signed the work order and returned a fully executed copy to the contractor. V. OPTIONS A. The Government has the unilateral right to exercise each of four (4), one (1) year options. The period of performance may be up to five (5) years if all options are exercised. Prior to exercising options, the Government will provide a notice of intent letter within 30 calendar days of execution. VI. SUBMISSIONS A. Each task order will establish a schedule for submissions and performance schedule. The performance schedule may be extended in the event performance is delayed due to causes beyond the control and without the fault or negligence of the AE as determined by the Contracting Officer. VII. SALARY RATES, OVERHEAD AND PROFIT A. Exhibit A-Architectural/Engineering Fee Schedule shall be used by the AE to identify costs associated with each of the multi-disciplines listed on the fee schedule. Upon award, the AE's proposed fee schedule for each discipline will be incorporated into the base contract, as well as each successive option year exercised. B. Exhibit B-Architectural/Engineering overhead and profit rate schedule shall be used by the AE contractor as applicable relevant to the total project design costs for each Task Order. Upon award, the AE's proposed overhead and profit rate will be incorporated into the base contract, as well as each successive option year exercised. C. The AE's direct/indirect costs, materials, and G&A expenses will be negotiated prior to award of the parent (IDIQ) contract, for each period of performance (base plus option years), for the life of the contract. D. Prior to issuance of task orders, the Government & Contractor will negotiate the number of labor hours required, site visits, construction period services and other incidentals. VIII. KEY PERSONNEL A. The AE shall employ the required professional personnel to perform the services required under this contract. No substitution will be made without the advance written approval of the Contracting Officer. The AE shall provide an explanation of the necessity for the change. No increase in salary rates will be allowed when personnel substitution is authorized. IX. CONTRACTING OFFICER'S REPRESENTITIVE A. With the award of each Task Order, the Contracting Officer will provide a delegation letter that will designate a Contracting Officer's Representative (COR) to assist in monitoring the work under this contract. The delegation letter will state the duties and limits of authority of the COR. The COR's duties include, but are not limited to: technical advice, substantive guidance, processing of invoices, and such other purposes as deemed necessary under the contract. B. **NOTE** The Contracting Officer is the only individual who can legally commit or obligate the Government for the expenditure of public funds, or change the terms and conditions of the contract. The technical administration of this contract shall not be construed to authorize the revision of the terms and conditions of this contract. Unless otherwise provided in this contract and subsequent Task Orders, the AE firm shall have the right to retain copies of all such materials beyond such period. (c) Warrant and Disclosure. The warrant and disclosure requirements of this paragraph apply with full force to both the contractor and all subcontractors. The contractor warrants that, to the best of the contractor's knowledge and belief, there are no relevant facts or circumstances which would give rise to an organizational conflict of interest, as defined in FAR Subpart 9.5, and that the contractor has disclosed all relevant information regarding any actual or potential conflict. The contractor agrees it shall make an immediate and full disclosure, in writing, to the Contracting Officer of any potential or actual organizational conflict of interest or the existence of any facts that may cause a reasonably prudent person to question the contractor's impartiality because of the appearance or existence of bias or an unfair competitive advantage. Requirements. The Medical Support Systems, Project Management Office (PMO-MSS) of the U.S. Army Medical Materiel Development Activity (USAMMDA) has a requirement to obtain a commercially available CPPW. This Statement of Need shall facilitate purchase of the CPPW to replenish stock. The proposed contract shall be Firm Fixed Price consisting of one base CLIN and two separate option quantity CLINs for the base year. 2. Background. The CPPW is used to protect patients who are incapable of donning individual chemical protective over garments. The CPPW will enable patients to be transported and treated safely during evacuation through chemically contaminated areas. It is used in forward areas on the battlefield such as Battalion Aid Stations and Medical Companies. The CPPW is also used with patients at Combat Support Hospitals. 3. Period and Place of Delivery Requirements 3.1. The proposed Delivery Date for the initial purchase of the Chemical Wraps is estimated at no later than 60 days after award. Earlier delivery is desired. The Period of Performance for the Option Quantity CLINS is estimated at 06 July 2016 - 05 December 2016. 3.2. Place of delivery. The primary delivery location will be at the U.S. Army Medical Materiel Agency Stock Maintenance Division, c/o U.S. Army Medical Materiel Development Activity (USAMMDA), BLDG. 1160, 6149 Wardleigh Road Bay 2, Hill Air Force Base, UT 84056-5948. 4. Minimum Essential Characteristics. 4.1. Protective Characteristics. 4.1.1 The CPPW shall be capable of providing respiration and percutaneous protection for unmasked, uncontaminated patients for a minimum of six hours after initial exposure to known potential chemical warfare agents that could be employed in vapor, aerosol, liquid, or thickened liquid form. The CPPW fabric, at minimum, must demonstrate resistance to permeation (liquid challenge) for two chemical agents: Soman (GD) and Sulfur Mustard (HD). Vendors must provide test reports or evidence of swatch testing of materials used in the fabrication of their candidate product. Aerosol Vapor Liquid Assessment Group (AVLAG) TOP 8-2-501 references standards for swatch testing of material. The test report must be provided from an independent lab or testing activity. 4.1.2 The upper surface of the CPPW will be water repellent so that the protective characteristics will not be degraded by exposure to moisture such as mist, rain, snow or saltwater spray. The bottom surface/material of the CPPW will be totally impervious to moisture, including liquid chemical agents. 4.1.3 The protective characteristics of the CPPW shall not be degraded when body excreta such as blood, urine and feces are brought into contact with the interior surfaces of the wrap. 4.2. Physical Characteristics. 4.2.1 The CPPW shall be a disposable, expendable item of medical supply and weigh no more than nine pounds in its packaged configuration, excluding the blower system, such as the Powered Air Purifying Respirator (PAPR). The current PAPR system used for this purpose is the C420 Blower and C2A1 Filter Canisters. Rechargeable batteries are required for use with the PAPR. 4.2.2 The CPPW shall be designed and manufactured in one size. It shall be large enough to completely encapsulate military personnel ranging from the 5th percentile female to the 95th percentile male, in accordance with MIL-STD-1472, and bear their weight without ripping or opening during movement. It shall have adequate space at the head to permit the use of a medical non-breathing mask, endotracheal intubation tubes, supraglottic devices, and cricothyroidotomy tubes. 4.2.3 The CPPW shall accommodate patients wearing current Army issue tourniquets, splints, backboard, and junctional hemorrhage control devices. 4.2.4 The CPPW shall be manufactured of materials which will not cause safety or health hazards to patients or using personnel. Vendors must provide safety or health hazard assessments (such as Material Safety Data Sheets (MSDS)) to indicate their product's assessment in this area. 4.2.5 Design will be such that air and carbon dioxide will be exchanged through the CPPW. Carbon Dioxide buildup shall be less than 3% (by volume) over a 6 hour test period in other than hot climatic regions as defined in Army Regulation (AR) 70-38. The oxygen level shall not fall below 18% (by volume) over the 6 hour test period in other than hot climatic regions as defined in AR 70-38. The hot climatic region is characterized by two daily weather cycles, one representing the highest temperatures likely to be found anywhere in the world and the other representing extremely high dew points. In hot climatic regions the time limit used will be determined during development, using healthy subjects. 4.2.6 A transparent window shall be located at the head-end of the CPPW to allow full head visibility of any size patient (5th percentile female to the 95th percentile male). 4.2.7 The fastening devices of the CPPW shall permit medical personnel easy and quick access to patients. 4.2.8 The CPPW must have an easily accessible, transparent pocket on its upper side. The pocket shall be able to hold a Patient Evacuation Tag (such as a Department of Defense (DD) Form 1380). 4.2.9 The CPPW shall be designed or marked in such a manner to prevent inadvertent reversing of top/bottom and ensure proper use. 4.2.10 The CPPW shall permit a patient to extricate him/herself without extreme effort within a time limit of 1 minute. It is recognized that in some situations the patient's injuries or sedation might make this impossible. 4.2.11 The CPPW shall allow for passage of intravenous/intraosseous infusion devices and respiratory tubing. Passageways should not exceed .59 inches in diameter. Two such passageways are required. 4.2.12 The CPPW shall have some type of carrying handles. The handles must be capable of holding military personnel ranging from the 5th percentile female to the 95th percentile male in accordance with MIL-STD-1472. 4.3. Packaging and Storage Characteristics. 4.3.1 Internal packaging shall be hermetically seam-sealed using available seam-sealed technologies for chemical protective items. 4.3.2 External packaging must be impervious to rain, dust and saltwater and shall permit rapid access to and employment of the wrap by medical personnel. Packaging materials must be impervious to all known threat agents for a period of time which is sufficient to permit decontamination by weathering. 4.3.3 The CPPW will be designed for storage in climatic design types hot and basic in accordance with Army Regulation 70-38 with no degradation of performance for a minimum period of 10 (ten) years. 4.3.4 Packaging dimensions shall not exceed 19.5 in long x 18 in wide x 6.5 in thick. 4.3.5 The blower system shall be able to pass limited rotary-wing Airworthiness Certification. 4.3.6 The government shall procure the CPPW as a kit comprised of 12 individual wraps. The kit shall include all the items (wraps, filters, blowers, batteries, etc.) necessary for each individual patient to have a complete CPPW system. The forecasted total procurement quantity is 492 CPPW Kits, which equates to 5,904 individual wraps. 5. Additional Requirements. 5.1 All items required to use in the kit in a deployed military environment shall be identified by part number, weight and cube to enable the establishment of National Stock Numbers (NSN). 5.2 Vendors shall include the following items with proposal: 5.2.1 Equipment Support Kit Lists. 5.2.2 Equipment Repairable Items List. 5.2.3 Equipment Commercial Service Manual. 5.2.4 Equipment Commercial Operation/Instruction Manual. 5.2.5 Equipment Consumable/Durable Support Items List. 5.2.6 Detailed Material Safety Data Sheet (MSDS) for all materials used in manufacturer and/or production of the CPPW.5.2.7 Certified report of swatch testing of materiel. 5.3 Any modifications to the Chemical Patient Protective Wraps during the Performacne of this contract that will affect future wrap deliveries, Contractor shall notify the Contracting Officer Representative (COR), Contracting Officer (KO), and Contract Specialist (CS) within 30 calander days of the modification completion. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due. Interested firms must submit three (3) copies and 1 CD of their SF330 no later than 1:30p.m. EST on November 20, 2015 to Bruce Caughman, Contract Specialist, Atlanta VA Contracting Office, 2008 Weems Road, Tucker, GA, 30084. Submission of information incorporated by reference is not allowed. Prospective firms shall address all selection criteria factors within their submitted SF330 packages. All submissions must be bound in some manner (no paperclips). Forms are available on the internet at http://www.gsa.gov/portal/forms/types/SF. The A/E selection criteria will be based on (In order of importance) (I) Professional qualifications necessary for the satisfactory performance of required services; provide key personnel, it is expected that the individuals identified as key personnel will be performing tasks under the resulting contract. Include resumes for key personnel and of proposed sub-consultants. (II) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction and the use of recovered materials. (III) Capacity to accomplish the work in the required time; maintaining project schedules and project budgets. (IV) Design Approach-Describe the proposed design philosophy; what problems do you anticipate and how do you propose to solve them? (V) Quality Control-What quality control techniques are planned? Who will be responsible for quality control? (VI) Past performance on contract with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules; be sure to include all past performance with Department of Veteran's Affairs (include knowledge of VA design standards, technical manuals, and specifications) (provide no more than three (3) recent projects). (VII) Location within 250 miles of the project area and knowledge of the locality of the project, please include a copy of your mapping results with your SF330 package; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project. (VIII) Reputation and standing of the firm and its principal officials with respect to professional performance, general management and cooperativeness. (IX) Record of significant claims against the firm because of improper or incomplete architectural and engineering services, and Insurance litigation (Type and amount of liability insurance carried, litigation involvement over the last 5 years and its outcome). (X) Specific experience and qualifications of personnel proposed for assignment to the project and their record of working together as a team. (XI) Awards (Awards received for design excellence). Firms will be evaluated by the A/E Evaluation Board, scored and ranked according to score. The three (3) (more or less) most qualified firms will be invited to interview with the selection board, and will again be numerically scored. The firm with the highest score will be selected and asked to negotiate a fair and reasonable price. Each respondent must be able to demonstrate the ability to comply with FAR Clause 52.219-14, Limitations on Subcontracting and all Joint Ventures must be CVE verified, submit agreements that comply with 13CFR 125.15 prior to contract award. This is a request for SF330's Architect-Engineer Qualifications only. This is not a request for proposal; no solicitation package will be issued.
