: Opracowanie dokumentacji projektowej na budowę kanalizacji sanitarnej, kanalizacji deszczowej, odwodnienia, sieci wodociągowej wraz z odtworzeniem pasów drogowych w Zelowie i Zelówku gm. Zelów. Opis przedmiotu zamówienia: Do obowiązków wykonawcy należy wykonanie i uzyskanie następujących opracowań i uzgodnień: 1. Wykonanie map d/c projektowych z oznaczonymi rzeczywistymi granicami pasa drogowego. 2. Wykonanie dokumentacji geologicznej. 3. Wykonanie karty informacyjnej i ewentualnego raportu oddziaływania na środowisko w postępowaniu o wydanie decyzji środowiskowej wraz z jej uzyskaniem. 4. Pozyskanie wypisu i wyrysu z planu zagospodarowania przestrzennego. 5. Pozyskanie wypisów z rejestru gruntów. 6. Pozyskanie warunków technicznych od administratora sieci tj. Przedsiębiorstwa Wodociągów i Kanalizacji sp. z o.o. i zakładu energetycznego. 7. Opracowanie projektów technicznych i wykonawczych w branży: - sieci kanalizacji sanitarnej wraz z odgałęzieniami do posesji ( pisemne uzgodnienie z właścicielami na mapach) i pompowniami wraz z zasilaniem energetycznym (w/z) z uzyskaniem warunków zasilania, - sieci kanalizacji deszczowej z odwodnieniem, - branża drogowa i odbudowa pasów, - kosztorysy dla poszczególnych branży inwestorskie i ślepe, - specyfikacje techniczne wykonania i odbioru robót. 8. Pozyskanie niezbędnych uzgodnień, opinii. 9. Wykonanie operatów wodno-prawnych wraz z uzyskaniem prawomocnych pozwoleń. 10. Uzyskanie w imieniu inwestora prawomocnych pozwoleń na budowę. 11. Uzgodnienie z zarządcami dróg, a w przypadku konieczności złożenie wniosku i uzyskanie zgody na odstępstwo od warunków technicznych/przepisów techniczno-budowlanych. 12. Wykonawca dokumentacji będzie, w ramach wynagrodzenia za wykonanie przedmiotu zamówienia, sprawował nadzór autorski do końca realizacji projektu, w ramach którego zobowiązany będzie do uzgadniania zmian i sporządzania dokumentacji zamiennych. 13. Przed złożeniem projektu do ZUD należy go uzgodnić w referacie Zamówień Publicznych, Inwestycji i Strategii tut. Urzędu. 14. Kompletną dokumentację wraz z prawomocnymi pozwoleniami na budowę należy dostarczyć do siedziby zamawiającego w terminie wskazanym w SIWZ. Zakres projektu w drogach gminnych i powiatowych: 1) Kanalizacja sanitarna i deszczowa: ul. Poprzeczna, Podleśna, Zofii, Górna, Leśna (do Wesołej), Przejazd, Pabianicka (domy przy Północnej), Bujnowska, Zelówek 2) Pompownie ścieków wraz z zasilaniem energetycznym i warunkami zasilania 3) Wodociąg - przedłużenie zaprojektowanej sieci w ul. Leśnej do ul. Wesołej wraz ze spinką, ul. Bujnowska, 4) Przy opracowaniu projektów należy uwzględnić posiadane przez Gminę projekty wodociągu w ulicach: Leśnej, Przejazd, Poprzeczna, Podleśna. Należy przewidzieć zaprojektowanie dodatkowych przyłączy w tych ulicach do działek budowlanych, do których nie było projektów wyprowadzeń w dotychczasowym opracowaniu. 5) Drogi - część drogowa dla Zelowa i odtworzenie po robotach na Zelówku. 6) Odwodnienie: przebudowa przepustów pod ul. Kilińskiego i Północną metodą bezwykopową, wraz z przykryciem kanału deszczowego wzdłuż ul. Jana Pawła II i włączeniem w istniejący kanał w ul. Wschodniej wraz z przejściem bezwykopowo pod ul. Dzielną. 7) Projekt docelowej organizacji ruchu uzgodniony z niezbędnymi uzgodnieniami. 8) Dokumentację projektową wykonać w 5 egz. dla inwestora oraz dodatkowych egzemplarzy niezbędnych do uzgodnień, decyzji i prawomocnych pozwoleń na budowę, kosztorysy inwestorskie 2 egz. w wersji papierowej, nakładcze 2 egz. w wersji papierowej, płyta CD - 1 szt. - kosztorysy w formie edytowalnej i pdf. 9) Kosztorysy sporządzić w rozbiciu na branże wod-kan, elektryczną i drogową osobno jezdnia i chodniki. 10) Kosztorysy osobno dla każdej ulicy. 11) Wszelkie kolizje wynikające z zakresu projektu należy rozwiązać w postaci dodatkowych projektów bez oddzielnego opracowania. Warunki techniczne 1) Kanały należy zaprojektować tak, żeby jak największa ilość działała w systemie grawitacyjnym. 2) Pompownie projektować bez nadbudowy, jako studnię okrągłą, przystosowaną do ciągu komunikacyjnego. 3) Rury do kanałów projektować lite. Zamawiający nie wyraża zgody na projektowanie kanałów z rur z rdzeniem spienionym. 4) Studnie dobrać wg uzgodnienia z użytkownikiem PWiKsp z o.o. w Zelowie 5) Przyłącza kanalizacji sanitarnej zakończyć studnią - do wszystkich działek zabudowanych lub w budowie, działki niezabudowane wyprowadzone przyłącza zakończyć korkiem. 6) Przyłącza wodociągowe zakończyć zasuwą w granicy działki. 7) Na mapie uzyskać pisemne uzgodnienia lokalizacji odgałęzień i przyłączy z właścicielami nieruchomości. 8) Przyłącza i wyprowadzenia należy zaprojektować do każdej działki budowlanej, 9) Projekt należy dostosować do istniejącej dok. projekt. w zakresie wodociągu w ul. Leśnej, Przejazd, Podleśnej, Poprzecznej. 10) W części drogowej na skrzyżowaniach zaprojektować krawężniki łukowe o znormalizowanych parametrach. 11) Kostka brukowa dostosowana kolorystyką do istniejących układów drogowych, 12) Wjazdy i niweletę dróg dostosować do istniejących posesji i ich wjazdów, Do każdej posesji dojście do furty i wjazd bramowy. 13) W przypadku kolizji elementów pasów drogowych ze słupami, drzewami, krzewami przewidzieć ich oznakowanie, przestawienie lub usunięcie. 14) Montaż studni w jezdniach przewidzieć w sposób gwarantujący należytą regulację do poziomu jezdni z pierścieniem odciążającym wg. wskazania producenta z włazami samopoziomującymi. 15) Wyposażenie pompowni do ustalenia i uzgodnienia z administratorem sieci tj.PWiK sp. z o.o. 16) Dokumentację projektową wykonać w 5 egz. dla inwestora oraz dodatkowych egzemplarzy niezbędnych do uzgodnień, decyzji i prawomocnych pozwoleń na budowę, kosztorysy inwestorskie 2 egz. w wersji papierowej, nakładcze 2 egz. w wersji papierowej, płyta CD - 1 szt. - kosztorysy w formie edytowalnej i pdf. UWAGI: Zakładany zakres uzbrojenia wynosi: - kanalizacja sanitarna (ok. 10,5 km) - kanalizacja deszczowa i odwodnienie (ok. 8,5 km) - wodociąg (ok. 1 km) - długość dróg do odtworzenia (ok. 10,5 km) Dokumentacja powinna zostać wykonana zgodnie z przepisami ustawy Prawo zamówień publicznych, w szczególności dotyczących opisu przedmiotu zamówienia. Przedmiot umowy będzie wykonywany w dwóch etapach: - I Etap obejmuje wykonanie i dostarczenie do siedziby Zamawiającego map do celów projektowych i w terminie do dnia 31 października 2014r. - II Etap obejmuje wykonanie i dostarczenie do siedziby Zamawiającego kompletnej dokumentacji projektowej wraz z prawomocnym pozwoleniem na budowę w terminie 1 roku od dnia zawarcia umowy. Warunki płatności: Pierwsza płatność nastąpi po wykonaniu i odebraniu bez zastrzeżeń I etapu zamówienia tj. wykonaniu i dostarczeniu Zamawiającemu map do celów projektowych, w terminie 30 dni od daty złożenia prawidłowo wystawionej faktury lub rachunku. Druga płatność nastąpi po wykonaniu i odebraniu bez zastrzeżeń II etapu przedmiotu zamówienia tj. po wykonaniu i dostarczeniu do siedziby Zamawiającego kompletnej dokumentacji projektowej wraz z prawomocnym pozwoleniem na budowę, w terminie 30 dni od daty złożenia prawidłowo wystawionej faktury lub rachunku..
