The objective of this PPI project is to improve the pharmaco-therapeutic process of patients who are dispensing drugs from the Outpatient Pharmaceutical Care Unit (UAF-PEX) of the University Hospital Miguel Servet of Zaragoza (HUMS) through the use of new technologies.

This project consists of the acquisition of a comprehensive service to improve safety in the use of drugs and ensure the adherence of chronic elderly patients (> 60 years). The target patients will be: onco-hematologic patients, HIV, Pulmonary arterial hypertension and chronic renal failure.

Permit the process management in a multidisciplinary way, bringing added-value from the different agents, pharmacy, patients associations, distribution companies, technological providers y telecommunication solutions.

Application for management and follow-up treatments

A digital backbone that embraces the whole process: from creating the prescription, validation, delivery, intake calendar generation, information sharing to patient.

Service Contract, which includes the implementation of a tool for managing the pharmacotherapy treatment process of the Ambulatory Dispensation Unit of the HUMS (Miguel Servet University Hospital), aimed at elderly patients and with special emphasis on chronic pathologies. The service includes the technological needs that favour the total automation and digitalization of the entire process, both from the point of view of the logistics of the drug, and from the point of view of prescription, dispensing and monitoring. Information management tools were incorporated to facilitate patient autonomy and adherence monitoring.

Everything necessary for its implementation within the space destined to this end should be included in the service, civil works, training, management tools and complementary services for automation and dispensing of medicines.
