Title of Project: Calibration Plan for audiology equipment. 2. Scope of Work: The contractor shall provide Calibration on all equipment every 6 months. 3. Background: The contractor shall complete all calibrations according to MFG requirements and specifications. 4. Performance Period: 06/01/2017-05/31/2018 2 services (one service every 6 months) 5. Type of Contract: Calibration Contract. 6. The contractor will have access to sensitive patient information. B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. Contractor has waived this meeting due to Urgent calibration situation. C. GENERAL REQUIREMENTS The contractor shall provide described parts and Labor. The contractor shall update the Clinical Engineering COR on the status of his work. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES The contractor shall complete all calibrations according to MFG requirements and specifications. E. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES [If applicable] Optional Deliverable One: None. SCHEDULE FOR DELIVERABLES 1. The contractor shall complete the Delivery Date column in Attachment A for each deliverable specified as soon as possible. 2. If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain reasoning to the CO, including a firm commitment of when the Parts and Labor shall be delivered. SPECIFIC TASKS: Vendor is required to deliver and install items requested in purchasing package. Delivery is audiology clinic room (Sound Booth 1 and Sound Booth 2) at the Savannah CBOC. Contractor is responsible to ensure delivery of goods and services as noted in procurement package and contact site representative when equipment has arrived for installation.
